Dear Parents/Guardians and Families,
La Grange School District 102 is excited to host a districtwide vaccination event in cooperation with Albertsons/Jewel-Osco. COVID-19 vaccines for children ages 5-11 will be available at the clinic. LTHS will also be hosting a clinic on the SAME DAY for families. They will provide vaccinations for 5-11-year-old children as well as for 12+ children and adults. In addition, they will provide boosters for those who qualify for boosters over the age of 18. Please make sure you are signing up for the correct clinic. The registration listed below will only be for the clinic held at Park Junior High for children ages 5-11. An additional email will have the links to the LTHS clinic
By creating more opportunities to boost the rate of vaccinated people in our area, our hope is that it will lead to a lower number of new infections, hospitalizations and create safer, healthier schools for all our students. Please consult with your family pediatrician if you have any questions or concerns regarding vaccinating your child(ren) between 5-11 years of age.
Any resident, 5-11 years of age living in the La Grange 102 community, can register to receive a free Pfizer COVID-19 vaccine at Park Junior High School on Saturday, November 13 (1st dose) and Saturday, December 4 (2nd dose).
Two steps are required:
Log in to the D102 Vaccination Clinic to make an appointment for November 13.
Complete the required Consent Form. Be sure to bring the completed form to the clinic.
For those registering for the COVID-19 vaccine, your November 13 appointment will be the same time of day as your December 4 second dose appointment time.

Parents, please bring the following to your vaccination appointment:
A copy of a valid photo ID (driver’s license, State ID, or student ID)
A copy of your health insurance card. If you do not have health insurance, you can still receive the COVID-19 vaccine at no charge to you.

Clinic participants can park in front of or behind Park Junior High School. Park Junior High is located on Ogden Avenue between Brainard and Park Road. The address is 325 N. Park Rd. La Grange Park, IL 60526. Please note that masks and social distancing are required at the vaccination clinic.
