Lyons Township High School District #204 is excited to host another vaccination event for all individuals who are eligible to receive the COVID-19 vaccination, including children aged 5-11. This vaccination event is hosted in cooperation with Albertsons/Jewel-Osco and is available to all LT Associate School Districts. The COVID Vaccine Clinic is set for Thursday, January 20, from 3-7pm at the North Campus Fieldhouse.
COVID-19 Pfizer pediatric vaccines and Pfizer/Moderna boosters will be available at the clinic via the links below. In addition, flu, shingrix, Tdap, and pneumonia will also be available and may be requested through the Booster link below.
By creating more opportunities to boost the rate of vaccinated people in our area, our hope is that it will lead to a lower number of new infections, hospitalizations and create safer, healthier schools for all our students.
Any individual, 5+ years of age and older, living in the Lyons Township High School District #204 community or within any of LT’s Associate School communities, can register to receive a free COVID-19 vaccine at LT's North Campus Fieldhouse on Thursday, January 20 (1st dose) and Thursday, February 10 (2nd dose). In addition, anyone in need of a COVID-19 Pfizer or Moderna booster, and who meets the criteria, may take part in the clinic. Two steps are required:
1. Log in to the Vaccination Site and make an appointment for January 20.
2. Complete the required Consent Form (Spanish version). Be sure to bring the completed form to the clinic.
For those registering for the COVID-19 vaccine, your January 20 first dose appointment will be the same time of day as your February 10 second dose appointment time.

Please bring the following to your vaccination appointment:
· A copy of a valid photo ID (driver’s license, State ID or student ID)
· For ages 5-11, please ensure the Date of Birth is included on the consent form and that the parent/guardian has signed the consent form.
· A copy of your health insurance card. If you do not have health insurance, you can still receive the COVID-19 vaccine at no charge to you.
· For participants receiving a Booster shot, they must bring their CDC card with them.

Clinic participants should park in the main parking lot off Cossitt Ave. across from LT’s North Campus (100 S. Brainard Ave., LaGrange). Please note that space will be extremely limited from 3-3:30pm as school dismisses. Parking on Sunset and Blackstone avenues is also permitted during clinic hours. Please note that masks and social distancing are required at the vaccination clinic.
